JOB SUMMARY:
A member of the community who applies his or her unique understanding of the experience, language, and/or culture to communicate a comprehensive health education program to diverse members of the community specifically in education partnership with parents, families, and children to complete behavioral-developmental screening. The Community Health Worker Intern will be responsible for assisting patients with a variety of care coordination activities. This role specifically focused on building an educational partnership with parents, families, and children to complete behavioral-developmental screening. Activities will vary based on patient need, but basic functions will include helping patients to navigate the healthcare system, understand and be connected to CentraCare entities to eliminate barriers to care. This position is temporary and will run from June 2024- August 2024 for a total of 300 working hours.
