VNA is dedicated to providing the highest quality health care to residents of the Central Coast. Our Home Health services focus on rehabilitation and restoration of independence after surgery, an acute illness, as well as management and stabilization of chronic diseases. Be part of a skilled team who assist patients, families and caregivers in the comfort and familiarity of their own home.
The Primary Case Manager works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers. Case manages patient care within the guidelines of the various payor sources. Prepares patient/family for discharge when appropriate, referring to community resources.

Education/Experience/Qualifications: Possession of a license to practice as a Registered Nurse in the State of California. Two years of experience as a professional nurse in a medical-surgical facility within the last three years and one to two years of recent home care experience. Preferred knowledgeable about all agency services. Possession of current CPR certification.
