JOB DEFINITION:

Social Worker position that will provide brief interventions and treatment to 2 Long Island JCC locations.


DUTIES AND RESPONSIBILITIES:

  • The Mental Health Professional, will spend 50% of his/her time at two locations.
  • Gather sufficient information for each client to make a diagnostic assessment necessary to determine referrals within CN Guidance,
  • Implement appropriate crisis services when needed.
  • Regularly assess with assigned clinicians at CN Guidance and the JCC SW staff the progress of clients' treatment and needs.
  • Exchange information with other agencies that benefits the client if needed
  • Knowledge of community resources that are culturally relevant
  • Educating and informing members informally about mental health, addiction, co-occurring disorders, stigma, fear, and ways to help oneself or friends/family members who may be struggling with any of these issues; and how CN Guidance can assist.
  • Providing on-site brief intervention screening and therapy (a few examples include administering surveys that help identify depression, such as PHQ-9; performing SBIRT [Screening, Brief Intervention, and Referral to Treatment]. Must be SBIRT trained.
  • Providing structured, community-wide presentations tailored toward various audiences;
  • Delivering structured, brief counseling sessions related to mental health and/or substance use disorders or concerns;
  • Offering access to crisis services via 24/7 phone number
  • All such services (including the screenings and brief therapies on-site) will be available to JCC members, community members from local synagogues, and schools linked to the JCC hubs.
  • Care coordination-helping people identify their full array of service needs (beyond behavioral health) and providers across the community who can assist in meeting those needs, and
  • Referral and "warm hand-off" (meaning that the liaison referring helps the community member to directly and fully connect with the agreed-upon specialist[s]). Such warm hand-offs will be to CN Guidance's clinic team members at our Certified Community Behavioral Health Clinic in Hicksville.
  • We will also provide ongoing/comprehensive mental health and substance abuse treatment at our Hicksville clinic to individuals. Services will include individual and group therapy, medication-assisted treatment, support groups, and education forums. Establish and maintains appropriate professional files demonstrating to the Agency, funding and licensing authorities appropriate therapeutic intervention with each client.
  • Reviews, develops and interprets quality assurance standards.
  • Maintains current regulatory guidelines for program and monitor program activities against these guidelines.
  • Prepares statistical and narrative reports as required.
  • Reviews client records, applying quality assurance and/or utilization review criteria to determine need for admission and continued stay in program.
  • Represents the Agency by serving on designated external committees or at meetings, reporting back on outcomes.

  • Other duties as assigned.


EDUCATION :

  • Master's Degree in Social Work
  • 2 years' experience of post-graduate clinical work in relevant settings including some experience with UJA funded programs.
  • Licensure in New York preferred
  • Required to apply for and maintain highest level of professional licensure upon eligibility.
  • Must be trained in SBIRT.
