Pay Range: $67,600 - $119,646 USD annually.

Purpose of Job:
Improve the overall health of the communities we serve by providing oversight of the Case Management team as follows:
Essential Duties and Responsibilities:
·Supervises, trains, and develops Case Management staff, encouraging teamwork and employee engagement.
·Provides program oversight to team.
·Ensures team meets or exceeds goals.
·Builds and maintains schedules for the team.
·Serves, manages, and supports internal and external partners.
·Leads department performance improvement workflow optimization and process improvement.
·Performs duties of a Case Manager.
·Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: Three years of experience within a community health setting required. Experience with case management preferred. Experience with electronic health records preferred. Case Management Certification preferred. Prior supervisory experience preferred.
Registered Nurse: Bachelor's or Associate’s degree in nursing from an accredited school of nursing is required. RN license in the state of Washington required upon hire and eligible to obtain RN license in Idaho.
Licensed Social Worker: Master’s in social work from an accredited university required. Valid state licensure as an Independent Clinical Social Worker or Licensed Advanced Social Worker. LCSW-A or LASW-A with equivalent case management experience will be considered.
Other Licensures- CDP, SUD-P or equivalent licensure with patient populations. Bachelor’s degree in related field with two years experience. Certification in Case Management required at time of hire.
Skills: Excellent customer service, oral and written communications skills required. Compassion for patients required. Must possess a valid driver’s license, insurance and vehicle. Must be able to work well in a team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. Computer skills required. BLS (CPR/AED) required. Precepts students as needed. Commitment to supporting a safe, respectful, equitable, and inclusive environment required.
Physical Demands:
While performing the duties of this job, the employee is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
