As the Chateau Girardeau Clinical Social Worker, you will be responsible for the psychosocial well-being of the Health Care Center residents through communication, assessment, intervention, networking, counseling, and teamwork to ensure the highest level of care is provided for our exceptional residents.

Key Job Responsibilities Include:
- Serve as liaison/advocate for residents living in the Chateau Health Care Center (HCC) and their family members, throughout various life-changing experiences, and adjustments.
- Administer social history assessments and review Resident Rights upon the resident’s admission to the HCC.
- Complete annual review of Resident Rights and advance directives with HCC residents.
- Administer the Brief Interview of Mental Status (BIMS) in the required timeframe for all residents in the HCC.
- Complete the identified sections of MDS and care plans in the required timeframe for all residents in the HCC; participate in scheduled interdisciplinary care plan mtgs.
- Administer depression scales, assess, intervene, provide counseling, and make recommendations, as needed, for a variety of mental health and emotional needs. (Serve in the same capacity, as needed, for assisted and independent living residents.)
- Serve as a professional resource to the Health Care Center Team (HCCT) in facilitating psychiatric consultations, referrals to community resources, and in managing psychiatric/behavioral crises experienced by residents.
- Serve as an education resource to the HCCT regarding resident psychosocial wellbeing. Work with any contract counseling programs by recommending referrals, as needed.
This is a full-time salaried position. Hours will vary based on business needs.
Qualifications/Requirements:
- Bachelor’s degree in social work and current license in Clinical Social Work
- Competency and work experience as a clinical social worker, licensed counselor or as practitioner in related field
- Comprehensive knowledge of social process, human development and motivation, and the aging process
- Knowledge of Federal, and State laws and regulations in long term care
- Effective interpersonal, verbal and written communication, and computer skills
- Comprehensive knowledge of community resources and services available to support residents
- Commitment to personal development and continuous improvement in knowledge and skill to more effectively perform duties of the position
