What are the responsibilities and job description for the Street Outreach Worker position at CHD Brand?
The Street Outreach worker is a professional who actively supports the philosophy and mission statement of CHD and CHD/Adult Mental Health, and who also maintains a high level of commitment to the outreach program. The Street Outreach worker oversees a variety of cases through different venues on a daily basis in the Waterbury and surrounding areas and is able to provide the unhoused seeking assistance the necessary tools to accomplish their goals.
Responsibilities
• Maintain a schedule including hours of actual street outreach, use of equipment and supplies as well as a schedule of external support providers that may able to further the individuals’ attempts to end their homelessness
• Help in maintaining all required records of the usage of the service provided and will meet all deadlines
• Work within the program’s model as well as protocols established by the Western CT Mental Health Network and the Coordinated Access Network with whom we jointly run the outreach services establishing open communication, support and accountability among collaborating agencies
Requirements
• Bilingual (English/Spanish)
• Must have a comfort level with being in the community and outreaching to client in locations such as the woods
• High school diploma or GED required
• Bachelor's degree from an accredited educational institution preferred
• Experience working with psychiatric disorders or co-occurring population preferred
