The Care Coordinator, IntuneHealth is a highly visible customer service and patient-focused role that is cross-trained to perform clinical duties under direct physician supervision and in accordance with state medical practice acts. The incumbent in this role works directly with our patient population and their families, insurance representatives, outside vendors, physicians, clinicians and other medical personnel to ensure the referral process runs smoothly. He/She operates in a dynamic and professional environment to help deliver VIP service to our patients.IntuneHealth is for Medicare Advantage seniors seeking PCP-led, high-quality care when and where they need it. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills, when necessary, all under physician supervision.
- Launches telehealth visits, guides patients through using the technology, and performs all digital exam procedures, whilst providing the same quality documentation tracking as in-clinic visits.
- Coordinates and processes patient referrals to completion with precision, detail and accuracy.
- Communicates with patients about all applicable internal and external appointment information for referrals and any physicians’/clinicians’ needs or outstanding items.
- Communicates with patient via chat, text, email or other means, whilst meeting IntuneHealth response time standards.
- Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.
- Prioritizes high risk patients, monitors stats and prioritizes late orders when needed.
- Uses web applications to generate authorizations (Availity, CarePlus, HealthHelp, NIA, ZocDoc and any other approved web IVR for authorization processing). Uses web applications and software to manage consultation notes from patients’ appointments.
- Prepares and participates during daily care team meetings. Provides updates on high priority patients referrals.
- Assists with checking patients in, escorting patients to exam rooms, checking patients out and scheduling follow up appointments.
- Assists with measuring and recording patient vital signs, patient intake interviews and medical history.
- Assists with cataloging and communicating patient needs to the PCP, Clinician, Specialist and/or other clinical personnel.
- Follows up on Home Health and durable medical equipment orders to ensure patient receives services ordered.
- Provides extraordinary customer service to internal and external customers and partners.
- Performs other duties as assigned and modified at manager’s discretion.
KNOWLEDGE, SKILLS AND ABILITIES:
Good business acumen and acuity
Sound knowledge and understanding of general clinical referrals functions, practices, processes, procedures, techniques and methods
Knowledge of medical terminology, CPT, HCPCS and ICD coding
Exceptional customer service skills with a passion for serving others
Exceptional organizational skills with the ability to effectively prioritize, multitask, be meticulous and complete work in a timely manner
Outstanding verbal and written communication skills
Skilled in operating phones, personal computers, software, and other basic IT systems
Comfortable with cross-functional training in referrals and back-office duties
Ability to communicate with employees, patients and other individuals in a professional and courteous manner
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software
Ability and willingness to travel locally, regionally and nationwide up to 10% of the time; flexible to work weekends as needed
Spoken and written fluency in English

EDUCATION AND EXPERIENCE CRITERIA:
- High School diploma or GED equivalent required
- Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e., foreign-trained physician, paramedic, EMT, etc.) required
- A minimum of 1 year of work experience as a Medical Assistant, Patient Care Technician or similar in a hospital, medical clinic or comparable environment required OR documented clinical rotation training may be substituted for work experience
- A minimum of 1 year work experience in customer service, patient referrals and/or insurance verification highly preferred
- Experience working with geriatric patients is a plus
- BLS for Healthcare Providers required
- EMR system experience preferred
- A valid, active driver's license in State of employment is required; position may require travel within the market
