Responsible for case management and coordination of resident clinical activities, incident report hearings, work with Clinical Manager on clinical issues. This is a full-time, benefit-eligible position with a schedule that primarily consists of daytime hours.
Correctional health services are provided to individuals involved in the criminal justice system through individualized plans of transitional support and treatment. These services are intended to assist the successful reentry back into the community and with families or other support systems. These individualized services include 24-hour residential treatment in a structured environment with individual/group services and counseling, all of which address health issues including mental illness and substance use disorders an other social needs related to employment , education, housing and the development of connections and coordination with other community supports and services.

Requirements:

Bachelor Degree in social, human services, criminal justice or behavioral science program required. LMSW or Master’s degree preferred. MCBAP certification desired.

Excellent driving record and current valid driver’s license.

A minimum of one year experience working in human services, corrections, or community based services. Three years social work experience with at least two years of group and case work experience preferred. Work experience may be substituted for academic studies exchanging one year of work experience in a related field for one year of academic education. Total work experience needed in lieu of the combination of education and work experience is five years or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above
Responsibilities:
- Case management responsibilities on assigned clients to include: biopsychosocial assessments, treatment plans, individual counseling, group counseling, case conference presentation, progress reports to referral sources, facilitate outside appointments, aftercare development, discharge planning and paperwork.
- Consult with the Clinical Manager in developing and maintaining the continuity of treatment and resident activities within the program.
- Coordinate and monitor resident activities within the program treatment model.
- Participate in case conference to discuss trends and dynamics of the treatment population as well as participate in clinical case reviews.

- Maintain records of resident progress in a timely manner.
- Provide disciplinary action on incidents of clients in consultation with Clinical
- Maintain therapeutic environment.
- Facilitate counseling groups as assigned.
- Other duties as assigned.
