Overview:
We are seeking a dedicated Case Manager to join our team. As a Case Manager, you will be responsible for overseeing and coordinating the care of clients in various programs. Your role will involve working closely with individuals in need, providing guidance, support, and resources to help them navigate through challenging situations.
Duties:
- Manage a caseload of clients referred from court or administrative channels
- Conduct assessments and refer clients to appropriate interventions or treatments
- Monitor and track client progress towards goals
- Prepare and deliver status updates, success reports, and other necessary documentation
- Conduct case reviews, write detailed reports, and maintain accurate client records
- Coordinate with referral agents, investigators, and other stakeholders as needed
- Schedule and oversee client participation in educational classes or treatment programs
- Provide crisis intervention and mentoring support to clients in need
- Supervise and guide clients through crisis situations effectively
- Ensure compliance with relevant regulations such as Section 8, Medicare, and childcare guidelines
Qualifications:
- Bachelor's degree in social work, psychology, counseling, or related field (Master's degree preferred)
- Proven experience in workforce development, crisis management, child welfare, addiction counseling, or related fields
- Strong understanding of Section 8 housing assistance programs and Medicare benefits
- Experience in childcare services or mentoring programs is a plus
- Excellent communication skills with the ability to interact effectively with diverse populations
- Ability to handle crisis situations with empathy and professionalism
- Strong organizational skills with attention to detail for maintaining accurate records
- Ability to work independently as well as part of a multidisciplinary team
- Prior experience in supervising staff or managing cases is advantageous
