Intake Specialist

POSITION SUMMARY

Summary: The Intake Specialist (IS) will act as the primary contact for potential patients as they schedule services for THRIVE Certified Community Behavioral Health Center (CCBHC). This person will be responsible for conducting the initial pre-screening over the phone, or in person, to determine eligibility for the program. The IS also collects and enters participants’ demographic information in the Electronic Health Record for the health center’s database and assists with reports and file maintenance. This person works closely with all of the THRIVE team members to keep an open line of communication on all services provided to patients. The IS also ensures the coordination with the Patient Services Specialist.

MINIMUM QUALIFICATIONS AND COMPETENCIES

• 1. High School Diploma/ General Education Degree and two years experience in customer service related employment.
• 2. Excellent organizational skills, independent and responsive with attention to details and ability to prioritize multiple tasks.
• 3. Ability to work effectively in team environment, excellent interpersonal skills with positive attitude and maximum discretion when handling sensitive and confidential information.
• 4. Customer service oriented.
• 5. Exceptional analytical and data entry skills with ability to operate computer systems and software such as Microsoft Word, Excel, Power Point, Outlook and Internet.
• 6. Willingness to work flexible schedules as deemed necessary including occasional weekends and evenings on rotation per team schedule.

Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, and other requirements established by CPLC from time to time.
