JOB SUMMARY:
Under general supervision this position functions as a direct service provider to clients in accordance with the designated funding contract. This position provides intensive Mentoring & Case management services utilizing the Transition to Independence Process (TIP) model which can include but not be limited to any and/or all of the following: Case Management, Coaching, Comprehensive assessments, Referrals to community services, Case plans, Instruction and trainings (one-on-one and in group or classroom settings), Facilitation of workshops, and support groups In addition, this position is responsible for outreach, intake and client management. This position will make home visits and/or visits in the field to clients enrolled in the program and work collaboratively with teammates to serve 300 youth throughout the year.
MINIMUM REQUIREMENTS / QUALIFICATIONS:
Education:
Completion of AA degree with major coursework in social services, Psychology, Child Development or related/relevant field acceptable. Graduation from an accredited four-year college or university with a major course in social services, Psychology, Child Development or a closely related field preferred.
Experience:
Two years of relevant experience working with children and families or social work case management. Bi-lingual desirable.
Description of Duties:
1. Represent the agency in a consistently professional manner, always modeling a commitment to the agency’s Mission and Values.
2. Develop and implement appropriate case plans in a consistent manner appropriate to the individual needs of the clients and in a way that ensures contractual goal achievement
3. Maintain client files consistent with program policy and assist with any related statistical and/or evaluation components.
4. Participate in training of TIP Model and incorporate into daily service implementation with clients.
5. Provide intense case management & mentoring to assigned clients.
6. Assume responsibility for case assessments, service/client plans, and reports for each client.
7. Maintain case files and database programs on a daily basis as required by CAPC and/or BHS and assist with any related statistical and/or evaluation components.
8. Provide concrete activities and education to strengthen the client.
9. Teach problem solving skills to enhance functioning and assist in setting and achieving goals.
10. Coach clients to act as advocates for themselves and their children (when applicable).
11. Establishes trust with families/clients through family centered support and encouragement.
12. Responds to crisis situations when appropriate. Effectively utilize/implement the Suicide Risk Assessment screening and procedures to ensure staff are safe.
13. Provide training, workshops and support groups to program participants.
14. Have a solid working knowledge of all the rules, regulations, and laws that apply to the contracts.
15. Have a solid working knowledge of strength-based case management, trauma informed practices, behavior interventions and therapeutic techniques.
16. Have a basic knowledge of mental illness diagnosis and co-occurring substance abuse disorders. Participate in trainings and complete assessment tools with clients to successfully identify underlying mental health needs that require attention; refer as appropriate.
17. Promote and maintain a sense of teamwork between assigned department and other CAPC staff, BHS, community partner agencies, and other individuals relevant to the successful case management of each case.
18. Network with other community agencies and participate, when requested, in community and health fairs and other events designed to promote CAPC programs.
19. Other duties, both program specific and CAPC related, as assigned.
