GENERAL DESCRIPTION:
The Family Assessment Worker is responsible for initial identification, referral and coordination activities for high-risk families of newborns. This position reports directly to the Family Assessment Supervisor.
SUMMARY OF ESSENTIAL JOB FUNCTIONS:
- Conduct in hospital or clinic outreach or screening duties as outlined in the MOU.
- Establish and sustain partnerships within the community.
- Attend identified community coalition and service group meetings as assigned.
- Receive and process daily, referrals from hospital personnel as well as other agencies.
- Maintain a referral organizational system.
- Schedule assessments meeting the needs of the family.
- Adhere to HFA standards regarding contacts, time frames, assessment rates etc.
- Complete quarterly FAW reports.
- Offer and administer the FROG Assessment virtually or in person, with all eligible families to determine risk level and need for services.
- Ensure referral to the Family Support Specialist for continued follow-up and support services.
- Creates electronic family file, uploads to FSS’s folders.
- Maintain client records in accordance with program and funding source requirements.
- Meet weekly with Family Assessment Supervisor to discuss referrals and narratives.
- Attend and participate in all scheduled Healthy Families seminars, conferences and in-service trainings and Parent Events.
- Make DCS referrals as appropriate
- Provide community presentations and community outreach activities.
- Maintain professional positive relationship with hospital staff and other collaborative agencies.
- Will assist program as assigned/needed when enrollment openings are limited.
- Perform QA tasks as assigned.
- Perform other related duties as assigned.
REQUIREMENTS:
- Minimum of a high school diploma or equivalent required. Bachelor’s degree or higher preferred.
- Location: must be able to provide direct service to the designated client base in the outlined service area.
- Maintain a functional and safe home working space with adequate internet connection for the job.
- Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage and dress code.
- Meet program/department’s performance and productivity standards.
- Attend required agency and program/department meetings.
- Adhere to the schedule agreed upon with the supervisor, to include occasional evenings and weekend hours.
- Follow and model CFR’s core values.
- Participate in PQI initiatives.
- Must be eligible and able to obtain a Level one fingerprint card.
- Must have a valid Arizona driver’s license, vehicle and meet agency liability insurance requirements.
- Ability to communicate effectively both orally and in writing.
- Knowledge of and sensitivity to individual/cultural differences.
- Ability to establish trusting relationships and interact effectively with staff, families and partners.
ABILITY REQUIREMENTS:
- Sitting for extended periods of time daily; standing and walking short distances.
- Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
- Ability to use a variety of office equipment and machines as referenced.
- Normal manual dexterity and eye-hand coordination required.
- Repeated hand-wrist movement required.
The nature of the position involves a fast paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice and the position requires quick response time and flexibility.
