Want to work in the community providing high-quality de-escalation, suicide prevention and assessment services to children in crisis situations, diverting inappropriate inpatient hospitalizations and stabilizing children for follow up care? Job Summary for Service Coordinator/Case Manager:

Under the direction of the Program Director, the Service Coordinator provides case management and mentor services to children and young adults with a history of mental illness, multiple treatment failures, and who are at risk of out of home placement, within a multidisciplinary team, utilizing a person-centered approach.

  • Delivers case management and mentoring services to clients within a team concept where each team member is recognized for their unique role in helping the client and family achieve better functioning.
  • Provides quality therapeutic services to a full caseload of clients, meeting or exceeding team productivity, grant, billing, and administrative standards.
  • Assesses the client and family’s needs and develops a service plan that will improve the child’s overall level of functioning.

Requirements

  

Education and Experience Requirements for Service Coordinator/Case Manager:

  • Bachelor’s degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related field which includes the study of human behavior and development.
  • A minimum of one year of experience working with children or adolescents with serious emotional disturbances and behavioral needs· Current FL driver’s license and good driving record plus reliable transportation with automobile liability insurance is also required.

Preferred Education and Experience for Service Coordinator/Case Manager:

  • Certified in Targeted Case Manager is preferred
  • Trained in wraparound, family systems, crisis intervention, teenager or young adult suicide prevention and trauma-informed care
  • Three (3) plus years of experience working directly with crisis level adolescent is preferred.
