What are the responsibilities and job description for the LPN-Licensed Practical Nurse position at Child Guidance Resource Centers?
CGRC’s SHAIP (Supportive Housing Aging in Place) program is hiring for a part time Licensed Practical Nurse. SHAIP is a supportive housing service in Delaware County designed to meet the needs of the older adult (ages 55 and older) priority populations. Residents may include, but are not limited to, medically fragile, individuals with mental illness, co-occurring drug and alcohol disorders, and intellectual disabilities. The residents of SHAIP do not meet criteria for nursing homes, but they do experience significant cognitive, medical, and mental health challenges and need living support.
Some job responsibilities include:
- Support with the Medline;
- Communications with pharmacy, treating practitioners, ancillary supports, etc.;
- Clarification on physicians' orders;
- Follow through with medical treatment;
- Medication Administration Record review,
- Update record of medications;
- Check in cycle medications;
- Reconcile medications at the beginning of the month;
- Check and document weights & vitals;
- Blood sugar testing for residents diagnosed with Diabetes;
- Communications with specialists & hospital personnel, as needed;
- Monitor of Fatal Five conditions (aspiration, dehydration, seizures, constipation, and sepsis);
- Completion of monthly assessments.
- Complete all required paperwork within specified timeframes;
- Complete annual mandatory training requirements;
- Participate in team meetings and training activities as required;
- Participate in required continuing education to ensure compliance with licensure requirements;
- Perform other medical-related duties as assigned or requested to support the SHAIP clients.
