As a Medi-Cal Case Manager, you will:

Formulate, implement, and monitor individual child/family development service plans in conjunction with treating team members.

Support families in following through with treatment recommendations made by therapists and psychiatric providers.

Connect families to recommended resources and help follow-through with recommendations made by team.

Support team in monitoring risk, as needed.

Function as primary liaison for family in linking to services and service providers at CHC.

Assess clients’ needs, strengths, and promote healthy child/family development practices.

Support providers in completing Medi-Cal documentation and paperwork (e.g., CANS)

Collaborate with treatment team and links families to services within the county.

Facilitate interagency transfers or transfers to other levels of care.

Conduct follow-ups and monitor service delivery and client progress.

Maintain confidential client records and files and completes required agency and Medi-Cal paperwork.

Document all chart contacts within 72 hours.

Maintain 58% productivity.

Administer evaluation surveys as required by contract.

Understand and adhere to the laws governing child abuse reporting.

Attend meetings and trainings necessary for professional development and as required by the agency or its funding sources.

Perform other related duties as required and assigned.
What we’re looking for:

Ability to qualify as a Mental Health Rehabilitation Specialist (MHRS) with the Mental Health Department.

Bilingual Spanish or Vietnamese/English.

Effective verbal and written communication skills.
About your background (education/experience);
-
BA/BS in psychology or related field from an accredited academic institution with course work geared towards child development and behavioral mental health.

At least two years’ experience with children ages 6-20 preferred, including parental support and coaching techniques; at least 2 years of experience in mental health settings serving children and families or an adjacent field.
Working Conditions:
Physical Requirements: An employee in this position may be required to occasionally lift, move and/or set-up stacks of documents, laptop computers, charts and other materials. Employee may also be required to work for extended periods of time on a laptop.
Special Job Requirements: To meet the needs of our clients, flexibility in work schedule is required. Most work will likely be in the afternoon/early evening.
Contact with Others: Frequent interpersonal interaction with peers, team members, leadership team, and outside contacts.
Compensation:
Range: $28.00- $33.00 per hour
Pay Type: hourly, Part time, Full Time

This pay range is subject to change and may be modified in the future.
