Job Description
General Summary:
The Complex Care Coordinator will assist the care management team through direct and indirect response to the health care management issues and needs of patients. Assists medical providers, behavioral health providers, hospital-based complex care staff, and interdisciplinary team members with community support services. Develops protocols and interventional strategies for discharge improvements after recommendations are assessed by multidisciplinary teams. In addition, assists patient families with identifying external resources needed such as entitlement programs to address needs of medical equipment, housing, transportation, food security, etc.
Education:
- Bachelor's degree (or equivalent knowledge) in social work, psychology
Experience:
- 3 years' related and applied experience in health care
Knowledge / Skills / Abilities:
- Exhibits guiding behaviors that reflect Children's values and support our mission and vision.
- Knowledge in complex care management as well as knowledge of entitlement programs.
- Strong communication, assessment and interpersonal skills so as to maintain strong, positive relationship with individuals of diverse backgrounds and experiences.
- Exceptional writing skills needed to complete forms, reports, case noting in electronic health record and other written materials.
- Demonstrated strong and creative problem solving skills.
- Strong organizational skills.
- Knowledge of Microsoft products and electronic health record documentation.
- Able to work effectively in a team environment.
Physical Requirements and Working Conditions:
- Normal office environment where there is no reasonable potential for exposure to blood or other high risk body fluids.
- Local travel for home visits and meeting at community based organizations
- Must maintain flexible schedule to meet requirements of caseload.
- Ability to work some evenings and weekends for programming and professional development opportunities as assigned.
