Job Summary


Chisago County Public Health has a supervisor opening in our Family Health Unit. This position oversees the Family Home Visiting program, Women, Infants and Children (WIC) and Child Teen Checkup program. Work schedule is Monday through Friday 8:00 a.m. to 4:30 p.m., with occasional evening or weekend for special events. We offer flexibility with your time off requests, reimbursement for mileage traveled for work, and ability for some remote work. The position will be located at the Health and Human Services building in North Branch, MN. Candidates must hold a registered nurse license and a public health nurse license. Performs difficult work supervising the operations and staff of the Public Health Division, delegating work assignments, providing leadership and guidance to staff, assisting in the planning and implementation of programs, monitoring and ensuring compliance with the department budget, completing required reports, and related work. Work is performed under the general direction of the Community Health Services Administrator.
Supervision is exercised over subordinate personnel.


ESSENTIAL DUTIES AND RESPONSIBILITIES

1) Plan and develop public health programs 25%

• Using the community health assessment, assess and monitor community health needs and assets on an ongoing basis for each area of public health responsibility.
• Plan programs using established Local Public Health priorities
• Assure that the ten essential public health services have been considered in determining the
mechanisms to address LPH priorities and achieve statewide LPH objectives.
• Research, write and submit grant applications.
• Research other programs and apply knowledge to health needs in Chisago County
• Develop and recommend policies and procedures
• Plays a leadership role in preparing for Local Public Health Accreditation
• Cognizant of population trends within Chisago County, with special attention to populations of special needs, understanding the internal and external resource needs of programming specifically designed to support and improve the status of these populations.

2) Implement & Supervise Public Health Programs 15%

• Assure compliance with applicable standards and program requirements.
• Delegate and coordinate program responsibilities and activities.
• Identify and obtain needed resources.
• Prepare and submit required reports, workplans, and budgets.
• Monitor progress toward program plans/goals.
• Assure that departmental polices are carried out and followed by staff.

3) Evaluate public health programs 10%

• Analyze program efficiency and effectiveness, using available data.
• Assures the integrity of the public health programs through understanding and utilization of division software programs and available state reports.
• Prepare and submit required reports (including annual report) on a timely basis.
• Develop and implement standards of public health practice.
• Develop and implement tools to evaluate specific programs.
• Monitor the health status of Chisago County residents.
• Utilize quality improvement processes

4) Supervise public health staff 20%

• Provide recommendations regarding recruiting, hiring and other personnel related matters.
• Assign individual work to employees in the Public Health division.
• Authorize personal time off. Sign time sheets.
• Provide/direct orientation, approve in-service training and staff development.
• Monitor and evaluate performance of assigned division staff on a regular basis. Meet at least monthly with each direct report. Conduct regularly scheduled staff meetings. Conduct annual performance reviews.
• Assures personnel are keeping caseloads up to date, with timely client contact, charting, billing, time keeping and case closures.
• Demonstrate good coaching and corrective measures for improvement of staff performance in a constructive manner. Appropriately document performance deficiencies, implementing corrective action plans under the guidance of the Community Health Services Administrator, and Administration.
• Assist staff in problem solving, prioritization of work and overall job performance.

5) Financial – 10%

• Demonstrates proficiency in development, management and oversight of miscellaneous grant and
division budgets.
• Is cognizant of and assists the Department in obtaining revenue sources that are supportive of the mission of the Department as a whole, and the Public Health division of the Department.
• Assures that the Local Public Health grant dollars have been used to support the essential local public health activities and address the local public health priorities and issues as identified by the community health assessment and action plan.
• Works and assists the Department Accounting Services unit in the preparation and submission of required expenditure and revenue reports for miscellaneous programs, including the submission of annual reporting for state and county purposes.
• Authorizes purchased services, and expenditures in accordance with departmental/program guidelines and within the approved annual budget.
• Prepare and submit budget/expenditure reports.


6) Community and County Coordination 10%

  • Promote community awareness/understanding of Public Health programs.
  • Provide consultation to community groups regarding Public Health. Identify other community services and integrate/coordinate services to prevent duplication.
  • Participate on approved national, state, and community organizations and committees.
  • Promote and demonstrate a positive management team perspective. Regularly and actively participate in meetings between departments.
  • Provide supervision for PHN student clinical experiences with Baccalaureate level Schools of Nursing to help assure future PHN workforce.

7) Public Health Preparedness 5%

  • Serve in a National Incident Management System leadership role as needed.
  • Be available 24/7 to respond to Health Alert messages and community emergencies.
  • Assure that health alert messages are disseminated to the appropriate entities and individuals in the county within established timelines.
  • Participate in Public Health Preparedness trainings (including ICS classes), drills and exercises. Participate in Emergency Operations Center (EOC) drills and exercises.
  • Serve as contact for the Minnesota Department of Health.


8) TEAM PARTICIPATION – 5%

  • Display constructive and positive attitude toward co-workers, peers and the public while conducting work duties for Chisago County.
  • Attitude and work behavior contribute to the success of the employees’ work team as well as the overall success of Chisago County in the accomplishment of the mission.
  • Work with the Department leadership to coordinate service delivery and planning in matters of Local Public Health Accreditation and all programs present and future.

9) Assume additional duties as assigned

  • Serves as backup to the Community Health Services Administrator, assuming all duties and tasks as needed to assure smooth operations of the programs offered by the Division.
  • Other duties as assigned by the Community Health Services Administrator and/or the Health and Human Services Director.
  • Represents the Public Health Division, the Health and Human Services Department and Chisago County in a professionally competent manner at all times.
  • Demonstrates ability to develop and present information in a professional manner to entities and groups with interest in matters of public health both internal and external to the Department.



Desired Minimum Qualifications


Desired Minimum Qualifications:

  • Bachelor's degree in related fields of program oversight and considerable experience, or a combination of equivalent education and experience. Current licensure by the state of Minnesota as a Registered Nurse by the state of Minnesota is required if assigned to Family Health Unit.

Special Qualifications:

  • Current licensure by the state of Minnesota as a Registered Nurse required if assigned to Family Health Unit.
  • Valid MN Driver’s License.
