Job Summary:

Chloeta is seeking a Optometrist to provide direct eye care services to patients in a rural community in Pueblo of Acoma, NM. This medical facility provides ambulatory and urgent care services through the following departments: dental, optometry, pharmacy, laboratory, radiology, mental health, medical social services, and public health nursing. The specialty services include internal medicine, women's health, obstetrics, and diabetes education and are available by appointment.

Supervisory Responsibilities: This position has no direct supervisory responsibilities but does serve as a coach and mentor for other positions in the department.


Job Classification: Full-Time


Duties and Responsibilities:

  • Diagnose visual and ocular problems through external and internal physical examination of the eye and adnexa; detection of neuromuscular impairment or anomalies of binocular function, treatment of visual disorders through prescription of corrective lenses and pharmacological means.
  • Treat unusual and/or complex visual and ocular disorders through the application of advanced optometric techniques.
  • Utilize laboratory and radiology services to evaluate ophthalmic and related systemic conditions.
  • Use all forms of topical and ophthalmic therapeutic medications, including oral therapeutic medications to manage pathology and trauma.
  • Perform minor surgical procedures including, but not limited to, anterior segment foreign body removal, corneal debridement, and suture removal.
  • Provide emergency eye care services on an outpatient basis, including diagnosis and treatment of ophthalmic trauma and pathology consistent with hospital clinical privileges.
  • Receive referrals from other health care professionals within the health center for specialty consultation and management of ocular disease and coordinates medical eye care with other physicians as appropriate.
  • Direct optometric technicians to assist in the clinical examination as appropriate.
  • Provide appropriate documentation on rendered patient care utilizing the electronic health record (EHR) system. The physician is responsible for utilizing the EHR system to obtain, dictate, electronically enter and assure preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Clinical data reports shall be completed in a timely manner and shall be maintained in compliance with Joint Commission (JC) standards.
  • Consult with Service Unit Optometrists as necessary for guidance on treatment, referral protocols and/or complex cases.
  • Other duties as assigned.

Education and Experience:

  • Required: Must possess a current, valid, unrestricted license to practice optometry in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract. For a license that was restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application will not be considered for the purposes of this contract.
  • Required: Must be actively Board Certified (ABCMO or AAOABO).
  • Required: Must have a current certification by the Optometry Residency Program.
  • Required: Doctorate in Optometry from an accredited by the Accreditation Council on Optometric Education (ACOE).
  • Required: Minimum six (6) months of experience in specialty area. There must be no lapse of services of no more than six (6) months prior to beginning tour of duty.
  • Required: Shall maintain a valid, current, full and unrestricted DEA license with all schedule types II through V and maintain certification throughout the term of the contract.
  • Required: Shall maintain a valid and current Cardiopulmonary Resuscitation (CPR) and Basic Life Support (BLS) certifications and are mandatory.
  • Required: Must possess a valid state driver’s license throughout the term of this contract.
  • Required: Must submit 3 professional references.
  • License verification and certifications must be provided at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • Work environment involves possible exposure to hazardous materials, such as chemical, blood and blood borne pathogens.
  • Must be able to lift 50 pounds at times and be able to bend, lift and pull.
  • This position requires extended periods of being on your feet and prolonged work sitting at a desk operating a computer.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.


Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
