Job responsibilities include:
- OASIS assessments
- Care Plan Implementation and Oversight
- Coordination of Care between agency disciplines and physicians
- Documentation in EMR
- Supervision of HHAs and LVNs
Job Qualifications
- At least 1 year of clinical experience required as an RN.
- Home Health experience is preferred.
