What are the responsibilities and job description for the LVN – Congregate SNF position at Choice Medical Group?
The LVN will report to the Director of Nursing and works in conjunction with the physicians/Medical Director, nursing staff, residents, families and other facility departments to provide optimum individualized nursing care for the residents of the skilled nursing facility – congregate care services.
QUALIFICATIONS
- Current and in good standing, and kept in good standing/current throughout employment, California State License as a Licensed Vocational Nurse required.
- Current BLS certification from American Heart Association, and kept current throughout employment.
- Minimum one (1) year experience in a skilled nursing facility or hospital environment preferred.
- Must have experience in patient care and ability to document patient care electronically.

- Must have effective communication, facilitation, interpersonal and professional diplomacy skills.
- Strong attention to detail with ability to manage multiple priorities while meeting deadlines.
- Able to Problem-solve, negotiate and demonstrate independent decision-making.
- Strong assessment skills and is resourceful in emergency situations.
- Strong communication skills in both oral and written form.
- Positive attitude.
- Maintain strong skills in diplomacy, professionalism and trustworthiness.
- Familiarity with Microsoft applications, which include the ability to learn new computer systems and applications.
- Ability to manage multiple priorities and projects.
- Detail oriented with desire for producing work with 100% accuracy.
- Excellent analytical and problem-solving skills.
