Social Services Assistant

The primary function of this position is to provide support skills to assist the resident in attaining or maintaining the highest practicable physical, mental, emotional, and psycho-social well-being in accordance with all applicable laws, standards and regulations. Responsible for ensuring that the medically related emotional and social needs of the resident are met and maintained on an individual basis.

The general purpose of the Social Services Assistant is to be a professional who works as a member of the interdisciplinary team to help residents and families understand and cope with the emotional and social problems that affect their health status, and to find and use the resources needed for a smooth transition from home or hospital to the nursing home.

Specific responsibilities of the position may include, but are not limited to, the following:

• Participates in interdisciplinary care planning meetings, as requested;
• Assist in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.;
• Advocates for residents by accessing community resources, making referrals to other agencies; consults with in-house resources;
• Assist in the development, administering, and coordinating of department policies and procedures. Keep abreast of current federal and state regulations, as well as professional standards;
• Assist in planning, developing, organizing, implementing, evaluating, supervising and directing the social services programs and activities;
• Maintains regular resident and family contact; demonstrates a passion for caring as evidenced by interactions with employees, residents, families, visitors and the community at large;
• Helps resolve concerns of the resident and/or family;
• Meet with appropriate facility staff concerning resident issues;
• Maintain satisfaction surveys in order to ensure that residents, families and visitors are treated with respect and compassion;
• Assists with discharge planning, development and implementation of social care plans and resident assessments;
• Assists in coordination and organization of psychological and psychiatric services;
• Assists supervisor with planning, developing, organizing, and implementing events;
• Performs other related duties as assigned by supervisor;
• Have ability to manage stress tolerance, pressure and anxiety.

Education/Certifications/Experience

• EDUCATION:  High School diploma or GED, with one year experience in social service or case work in long-term care.   Associates degree preferred.
• LICENSURE/CERTIFICATION:  none required
• EXPERIENCE:  Entry level knowledge of the professional principles and skills used in the practice of social services; professional empathy in dealing with residents and families; sense of professional identity while collaborating with co-workers; awareness of community resources and programs for the disabled and elderly; appropriate professional judgment, courtesy and tact.

$17-20/ hour based on experience
