What are the responsibilities and job description for the Case Manager 3916 position at CHR?
To decrease oppositional behaviors, behavioral health symptoms, substance use, and maintain at risk
youth within their communities through outreach support, direct assistance and training in living skills and
advocacy.
By:
- Providing youth with behavioral health issues and their family members intensive home based
case management services and support - Alleviating family stress and burdens by advocating for clients and families.
- Helping family members link with outside resources to address basic needs of daily living.
- Engaging the youth in prosocial activities in the community
- Linking with appropriate treatment sources and providing wrap around care including school
collaboration
DUTIES & RESPONSIBILITIES:
- Understands specific model and uses concepts when appropriate to aid clients in understanding
and managing their symptoms. - Maintains family and other collateral contacts to ensure quality, client-centered services.
- Assesses and involves natural supports in treatment planning and discharge processes whenever
possible. - Provides case management, clinical, and crisis intervention supports as indicated. Works in close
collaboration with assigned clinician in event of crisis. - Insures that self- administered urinalyses is completed on a weekly basis if required by model
- Demonstrates competency in topics of child development, substance use, and provides trauma
sensitive and culturally competent care. - Uses EMR to document all services and contacts with clients, collateral contacts and
appointments. - Attends all model required meetings and agency trainings.
QUALIFICATIONS:
Education: Bachelor’s degree in human services required.
Experience: At least two (2) years experience in a mental health setting preferred. For identified
positions, bilingual capacity may be required.
Licensure/Certification/Registration: Valid driver’s license.
Training: Will attend all required program trainings.
