Description
As a medical social worker at CHCC, you will coordinate the admissions process and provide social services to residents and their families. You will assist them with emotional, spiritual, cognitive and financial issues in a professional, well-respected skilled nursing center.
Responsibilities
- Receive and document all potential facility admission inquiries.
- Communicate with facility departments regarding pending and scheduled admissions.
- Welcome and perform an initial assessment of new residents within 24 to 48 hours of admission.
- Provides backup to the admission coordinator as needed.
- Complete all assigned sections of the MDS within specified timeframes.
- Collect information to formulate the social history and assessment; contribute to the resident care plan. This may include interviewing family, friends and community agency representatives and utilizing clinical records.
- Assess resident Medicare eligibility, facilitate the completion of forms. Act as a liaison with DSHS financial workers as needed.
- May accompany nursing department on home pre-admissions assessments as requested.
- Ensures daily observation and assessment of each resident; directs and participates in resident care.
- Contributes as a member of the interdisciplinary resident care plan conference.
- Identifies specific issues and advocates for residents to contribute to their overall care and well-being.
- Attends and participates in psychotropic drug management.
- Initiate and/or participate in the development of written discharge plans.
- Ensures facility policies and procedures are followed and updated as appropriate.

Qualifications
- Bachelor’s degree in social work or three or more years of experience working as a social worker.
- Experience in a long-term care setting preferred.
- Understanding of the MDS process and general knowledge of long-term care Medicare rules.
Skills
- Knowledgeable and supportive of CHCC mission and philosophy.
- Demonstrated interest and ability to work with and understand the needs of institutionalized adults and developmentally disabled populations.
- Knowledge of state and federal regulations pertaining to long-term care.
Strong problem-solving skills, able to identify resident and family issues and assist in resolution.
- Excellent communication skills (verbal and written) and ability to work with diverse populations.
- Works alongside employees to promote a team effort and maintain the highest quality resident service.Job Benefits
