About Us

We are a growing organization that oversees the care for children and adults with intellectual disabilities (Downs Syndrome, Autism, Cerebral Palsy, etc.). Our network currently consists of five ICF care homes. One in Sacramento and four in Merced. Each home is designed to provide person-centered care for just six individuals. This design allows us to ensure true person-centered care.

Your Role

We are searching for an ambitious individual to grow with our organization. This is an ideal position for a person that desires growth as a clinical leader. The nurse would work on the sites 4 days a week and work remotely 1 day a week.

Since starting 3 years ago, we have grown from one to five locations and have plans to continue growing. As we grow in the next few years, the candidate that is chosen for this position will have the chance to become a nurse executive and supervise RNs.

An ideal candidate would be capable of identifying areas of opportunity and creating systems to improve the organization. This candidate would be able to audit care provision at each home and provide direction for improvement to caregivers and LVNs.

The RN will be based in Merced, CA and will be required to visit each Merced home Daily

Registered Nurse (RN)

The functions of the role:

  • Lead a team of LVNs and CNAs/ Caregivers
  • The Registered Nurse will participate in the development, review, and update of the Individual Service Plan as part of the Interdisciplinary Team (IDT) process
  • Develop, implement and train staff on nursing/nursing related care plans as part of the ID team
  • Review all related nursing services on at least a monthly basis and document the review
  • Revise nursing/nursing related care plans on an as-needed basis
  • Assess competency of unlicensed staff in nursing related/specialized procedures/medication administration implementation at a minimum of every 3 months
  • Will complete a quarterly (or more often as indicated) direct physical exam/assessment/systems review of each individual. If there are abnormal findings, the nurse will make appropriate referrals to the physician/specialists and maintain close communication with the QIDP and other pertinent IDT members
  • The nurse will actively promote health and wellness and ensure nursing actions and steps are put in place for prevention (such as if an individual has a history of skin integrity issues, padding at pressure points will be put in place)
  • Training staff and individuals (as relevant) in health issues, how to care for health needs and conditions, personal hygiene, health maintenance, and disease prevention
  • Provide instruction to staff in methods to control infection and, will maintain the infection surveillance log which will be reviewed with the Administrator, QIDP, and other relevant staff and team members on a monthly basis
  • Attend and participate in the quarterly Infection Control Committee meeting
  • Review the medication administration records at least every 2 weeks for compliance
  • Complete Drug Regimen Reviews 8 times a year/2 months of each quarter

The Registered Nurse will be involved in ongoing discharge planning assessment of the individual in relation to nursing needs.
