What are the responsibilities and job description for the Intake Case Manager-F&C position at City of Hampton?
The purpose of this class is to Perform advanced complex duties conducting the initial and ongoing eligibility determinations for financial assistance, SNAP (Supplemental Nutrition Assistance Program), Medicaid ( Aged, Blind and Disabled, Community Based Care and Long Term Care), and Auxiliary Grants. Requires advanced program knowledge and proficiency in the Virginia Case Management system. Works independently in accordance with well-established guidelines and standards following federal and state programs and procedural policy for public assistance programs. Promotes self-sufficiency for those customers seeking or receiving public assistance and refers customers to other community resources as appropriate.
Minimum Requirements
- Graduation from High School or GED. Two years of college coursework reflecting oral and written communication skills, time management, and the ability to solve math formulas and averages. Bachelor’s degree preferred.
- Two (2) years of experience as a Benefits program specialist or equivalent experience.
- Considerable experience in administering human service delivery and/or financial benefit programs such as TANF (Temporary Assistance for Needy Families), SNAP (Supplemental Nutrition Assistance Program), Medicaid, Child Care, Energy Assistance, or other benefit programs.
- Experience working in a team and/or customer service setting is required.
- Prior experience working with Human Service agencies and/or experience using the VACMS/MMIS computer systems.
- Must possess a valid driver’s license and must have and maintain a satisfactory driver’s record based on the City of Hampton’s criteria.
- Must successfully pass a background check related to this position prior to any offer of employment or promotion.
Additional Requirements
- Maintain professional ethics related to confidentiality, and exercise sound judgement. Must have the ability to work remotely and independently depending on the needs of the agency.
- Ability to operate a personal computer and VDSS software. Time management, competency-based interviewing, good listening skills, problem solving, and developing and maintaining positive professional relationships with all customers internal and external.
- Spanish, bilingual skills helpful. Must achieve a passing score, per agency guidelines, on policy training and tests in all major program areas before completion of the one-year probationary period.
- The incumbent may be considered “essential personnel” during city emergency situations, or at the direction of the City Manager or designee which may include long hours and unusual schedules.
