- Provide support and assistance to individuals in need, focusing on social work, behavioral health, addiction counseling, crisis management, crisis intervention, mental health counseling, motivational interviewing, systems, and mentoring.
- Conduct peer group and assessments to determine the needs and goals of individuals seeking support.
- Develop and implement personalized care plans to address the unique needs of each individual.
- Collaborate with other professionals and agencies to coordinate services and resources for clients.
- Offer counseling and guidance to individuals facing various challenges in their lives.
- Assist individuals in accessing community resources and support networks.
- Advocate for clients' rights and ensure they receive appropriate care and services.
- Maintain accurate records of client interactions and progress.
