Responsibilities:

Lead the inter-disciplinary team in Resident Care Conferences to ensure the social, psychological, and emotional needs of each resident and family member are met.
Oversee appropriate assessment prior to Initial Care Conference according to Minimum Data Set guidelines.
Maintain ongoing MDS assessments and care plans in related areas.
Complete documentation in every resident's record through care plan reviews and pertinent daily recordings.
Provide direct social work services to residents and family members, including counseling, group work, problem-solving, information and referral, financial assistance, legal assistance, and advocacy to facilitate adjustment to the facility.
Carry out social service duties in accordance with current federal, state, and local standards, guidelines, regulations, and established policies and procedures.
Ensure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.

Specific Requirements:

Must be able to read, write, speak, and understand the English language.
Possess the ability to make independent decisions when circumstances warrant such action.
Demonstrate tactful interpersonal skills when interacting with personnel, residents, family members, visitors, government agencies/personnel, and the public.
Exhibit patience, tact, a cheerful disposition, and enthusiasm, while adjusting interactions based on the maturity level of residents.
Be open to seeking new methods and principles to incorporate into existing Social Service practices.
Communicate relevant information regarding a resident's condition/incident to the Interdisciplinary Team as appropriate.

Experience:

Minimum of one (1) year of social work experience in a hospital, skilled facility, or other health-related field preferred.

Education:

Must possess, as a minimum, a BSW from an approved school of Social Work.
