Job Summary

This position works within the Healthy Communities Team to support the implementation of policy and program priorities identified as part of the Raising Clark County project.

The Family Initiatives Coordinator works with community partners to advance family support systems that help Clark County families thrive.

The position requires strong facilitation skills and the ability to work both independently and collaboratively with other CCPH staff, external partners, and community leaders.

This position reports to the Chronic Disease Program Manager II in the Lifecourse section of the Healthy Communities division

  • Other responsibilities include understanding and promoting the public health mission of the department; providing courteous, respectful, efficient customer service to all colleagues and partners;
  • honoring diversity of all department employees and constituents; participating in department training and applying what is learned in our work;

and striving for personal excellence in public health.

Qualifications

Education and Experience : Incumbents must demonstrate experience working with diverse community members, group facilitation, and program coordination.

All combinations of education, experience and training that demonstrate the ability to perform the work will be considered.

Master’s degree in public health and project management certifications preferred.

Knowledge of :

Prepare effective correspondence and reports.

Respond to public health emergencies or exercise at any time, except while on scheduled vacation or other leave. In addition, leaves may be cancelled under public health emergencies.

JOB FUNCTION : Program Coordination (FTE : 0.35)

Facilitates implementation and management of work plan activities using evidence-based best practices in collaboration with staff and community partners.

Ensures workplan activities and timelines / outcomes are completedor adjusted as needed to meet the evolving needs of the community.

Leverages subject matter expertise and best practice research to inform program and policy recommendations.

JOB FUNCTION : Community Partnership and Engagement (FTE : 0.35)

Engages internal and external partnerson all activities related to the Raising Clark County project and related Community Health Improvement Plan priorities.

Identifies, develops, engages, and maintains strategic partnerships with local and statewide organizations, associations, family-centered agencies, community groups, and government agencies to increase access and services of public health importance.

JOB FUNCTION : Community Capacity Building (FTE : 0.15)

Identifies and supports coordination needs for community partners with interdependencies to maximize the administration of allied health programming (i.

e.additional important services).

Works with partners to advocate for high priority policy-, system-, and environmental change initiatives for pregnant and parenting families, chronic disease prevention, and access to care.

Seeks funding to implement evidence-based or innovative prevention initiatives in collaboration with community partners.

Makes training opportunities available to advance understanding of the social determinants of health and the resulting impacts of systems of care on family and child health services, chronic disease, and access to care frameworks.

JOB FUNCTION : Additional Duties (FTE : 0.15)

Participating in departmental, community, and state-level committees / workgroups / trainings

Performing other staff duties and tasks as assigned.

Work is accomplished by :

Using qualitative and quantitative data to facilitate the community-driven selection process of identifying and implementingRaising Clark Countypriorities.

Effectively framing systemic issues using key data and experiences related to pregnant and parenting families, including epidemiological, economic, political, and social trends

Collaborating with community partners to implement evidence-based and innovative strategies and policiesthat support families and improve health outcomes.

Prioritizing primary prevention programming and community capacity-building.

Supporting the development of local communities of practice with allied organizations and community partners.

Thriving community partnerships

The focus areas of this position are :

Facilitating the implementation of family-centered policy and program efforts with community partners and the Healthy Communities team

Capacity Building with community partners (i.e., providing technical assistance and grant writing support for policy and program priorities).

Creating and maintaining relationships and strategic partnerships with allied pregnant and parenting family and early childhood community agencies, schools, businesses, and healthcare partners with an emphasis on closing gaps in health inequities and reducing adverse childhood experiences.

Retirement Information
