Purpose of Position:

The Case Manager will function as a multidisciplinary team member providing case management and service coordination as assigned.

Minimum Training and Experience Required to Perform Essential Job Functions

A Bachelor’s Degree in Social Work or a Human Services related field is required. A minimum of two years of experience working with individuals with developmental/intellectual disabilities, mental illness and/or substance use disorders is preferred. An equivalent combination of education and experience will be considered.

If applicable, the candidate must obtain and maintain certification to administer the Children’s Long Term

Support Functional Screen and/or the Mental Health/AODA Functional Screen within one (1) month of hire.

Candidate must possess a valid driver’s license and own a reliable vehicle.

Essential Duties and Responsibilities

The following duties are normal for this position; however, these are not to be construed as exclusive or all- inclusive. Other program specific duties may be required and assigned.

  • Assess and determine program eligibility (functional and financial)
  • Assess and identify individual service needs
  • Develop and monitor consumer driven service plans, including six month reviews and updates
  • Prepare and complete required documentation with proficiency and in a timely manner
  • Meet client and collateral contact standards as set forth by program standards
  • Provide outreach services, as applicable
  • Manage crisis intervention services, as applicable
  • Work collaboratively with other professionals, including but not limited to, school personnel, hospital/clinic staff, social workers, law enforcement agencies, service providers, etc.
  • Serve as back up support to other case manager within the department
  • Attend and participate in advisory committee meetings as assigned
  • Maintain professional working relationship with clients, outside agencies, county department, the general public and supervisors and co-workers
  • Maintain confidentiality and adhere to HIPAA rules and regulations
  • Follow Department polices/procedures and adhere to the Clark County Employee Handbook and

  • Maintain professional appearance and demeanor

OTHER SKILLS AND ABILITIES

Knowledge:

  • Knowledge of and familiarity with Wisconsin State Statutes relevant to assigned programs, i.e. DHS 34,

DHS 35, DHS 36, DHS 63, DHS 75, DHS 90, Chapter 51, Chapter 54 and Chapter 55

  • Experience working with individuals in a community based setting
  • Knowledge of and familiarity with Wisconsin Medicaid, as well as targeted case management (TCM) rules/regulations
  • Knowledge of local providers and community resources
  • Knowledge of evidence based practice, professional boundaries and ethics
