Our Program Manager works with the Clinical and Executive directors to oversee day to day operations of the Health Home Care Coordinator Program including the supervision of Care Coordinator. This position provides supervision to Care Coordinators working with clients with complex health needs, living independently or in assisted communities.

What we’re looking for:
- Bachelor or Master’s degree in a Human Services or Social Services or related field
- 3 years or more of successful work experience under minimal supervision
- Three years of experience and ability to work under minimal supervision
- Proof of: valid driver’s license, auto insurance, and education
