Self Direction Assistant (SDA) - Home Based Services
Program: Home Based Services
Location: Downers Grove, IL
Hours: Full-time, Monday - Friday, 9AM - 5PM, 40 hours/week
Pay: $53,380/yr
Clearbrook is a non-profit, social services agency that specializes in providing innovative services to individuals with intellectual and developmental disabilities, such as Autism and Down Syndrome.
The Intake SDA reports to the Intake Coordinator and is responsible for initial intake meeting (provide information of HBS services), obtaining required documents (service agreements, EOR, risk assessment, Aces$, etc.) and writing the Implementation Strategy Plan for the incoming HBS clients. The person must have in-depth knowledge of the Home - Based program. Additionally, this person must be willing to maintain a caseload as to stay up to date on the latest development in the home based program. This person will assist the director with the quality portion of the program through chart review, family discussions, and program tracking.
Qualifications
The SDA must have:
- Bachelor's in human services or similar is the minimum education requirement for this position
- At least one year of experience working directly with individuals who have intellectual / developmental disabilities
- Driver's license and valid car insurance with applicant's name on declaration page
