What are the responsibilities and job description for the Clinical Care Coordinator position at Clearway Pain Solutions?
The Clinical Care Coordinator monitors patient portals and responds to urgent readings within predesignated timeframes. The CCC will be the main point of contact for an assigned group of customers/patients.
Essential Duties and Responsibilities:
- Performs job in accordance with Company Mission, vision and goals.
- Accurately logs patient information into database
- Ensures patients are assigned and set up correctly in their portal, making corrections as necessary.
- Confirms the receipt of fee slips for tracking purposes
- Updates reports as necessary
- Engages in customer support by answering device questions
- Contacts patients directly regarding blood pressure readings
- Monitors portals for crisis readings and responds within designated timeframe
- Participates in other customer service activities as assigned.
- Checks work e-mail on a regular basis throughout the workday.
- Participate in and complete all required trainings and in-services.
- Performs other duties as assigned.
Minimum Qualifications:
- High School Diploma or GED.
- To perform this job successfully, an individual should have knowledge of Internet and Microsoft Office software (MS Word, MS Excel, MS PowerPoint, MS Outlook).
- Must have excellent written and oral communication skills, including exceptional customer service.
- Must be able to establish and maintain effective working relationships with doctors, clinical staff, other co-workers and the public.
- Must be able to work individually as well as within a team.
- Must be able to follow both verbal and written instructions.

Preferred Qualifications:
- Associates Degree
- One year (1) of prior experience in the medical field
- One (1) year of prior experience working with an Electronic Medical Record (EMR).
Driving/Travel:
The employee must have reliable transportation. Travel for this position will be required up to 25%. While the primary workplace may be closest to the employees home, work assignments could be in any of the Companys locations.
