Access Nurse

Access Nurses coordinate patient flow through facilitating hospital transfers and direct admissions, determining daily bed resources for all access points to the hospital including hospital transfer, direct admission, post-operative and emergency department admissions, and overseeing bed management logistics for the health system. This position is vital in ensuring a seamless transfer for incoming and outgoing patients.
A caregiver in this position works 7:00AM – 7:30PM or 7:00PM – 7:30AM on a PRN basis. You will be required to work at least 48 hours every 6 weeks including one weekend. After orientation, this caregiver will have the opportunity to work remotely with the exception of in-person training, computer issues or management requests. The caregiver in this role will be required to live within one hour of Cleveland, OH.
A caregiver who excels in this role will:
Serve as the primary point of contact for telephone patients with a variety of health or illness questions.
Utilize effective and professional communication, clinical knowledge, and nursing assessment skills to quickly triage patient status.
Provide intake, assessment, and triage for internal and external hospital transfers and direct admissions on request in accordance with standard operating procedures. Standard operating procedures include utilization of telephonic interaction as the primary mode of contact through recorded conference calls, completing electronic documentation, and connecting appropriate providers to ensure safe exchange of patient information for acceptance to the health system.
Elicit information from referring providers, review patient clinical information, and assess patient status for appropriate placement.
Collaborate with health care providers and hospital leadership to optimize patient outcomes utilizing clinical expertise and incorporating nursing and management experience with critical thinking skills & business intelligence tools to achieve optimal outcomes both clinically and financially for the patients and the organization.
Apply solid understanding of the complexities of the patient status and the process and capabilities of each system hospital to seamlessly facilitate and achieve enterprise bed placement.
Provide oversight of all bed management functions.
Function at any point in the patient flow process designated to ensure the safe and efficient placement of patients and effective utilization of resources.
Demonstrate cost effective management of enterprise bed resources to enable the organization to provide quality care to patients.
Minimum qualifications for the ideal future caregiver include:
Lives within one hour of Cleveland, OH.
Graduate of an accredited school of professional nursing.
Current state license as a Registered Nurse (RN).
Basic Life Support (BLS) certification through the American Heart Association (AHA).
Three years of recent progressive clinical experience in an acute care setting.
Preferred qualifications for the ideal future caregiver include:
Bachelor of science in nursing (BSN).
Two years of supervisory experience in a healthcare setting.
Computer proficiency in Microsoft applications, bed management databases and Epic.

Physical Requirements:
- Requires sitting for long periods of time, multitasking, manual dexterity to operate computers and normal or corrected vision.
- Requires working under stressful and emergency situations with the ability to maintain composure in any situation in order to provide superior customer service and ensure proper patient safety and placement.
- Must be able to perform multiple functions simultaneously without supervision and learn new practices promptly.
