Clewiston Nursing and Rehabilitation Center is now hiring an experienced MDS Care Plan Coordinator who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing Services, the MDS / Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

QUALIFICATIONS:
- Organizational Skills/Ability to Multitask
- Knowledge of PDPM/Clinical Concepts
- Ability to work with facility nursing management to identify and fix care documentation notes
