A leading acute care hospital located in southeastern New York is now searching for an experienced Cancer Support Social Worker to join their award-winning healthcare team.
This acute care facility was founded over one hundred years ago and today houses nearly 400 licensed beds and offers a wide variety of medical and surgical services. This state-of-the-art medical center provides a number of patient care services including behavioral health, oncology services, cardiopulmonary and vascular services, emergency and critical care, radiology services, maternity and pediatric care, wound and home care, and much more.
As part of an interdisciplinary care team, the Behavioral Health Social Worker will coordinate referral development and pre-admission patient assessments for potential clients. Other responsibilities include evaluating patients within the infusion center and inpatient oncology department, providing patients and families with education and advice, and addressing emotional, physical, or financial problems related to diagnosis. The Cancer Support Social Worker will also offer crisis intervention, bereavement, client advocacy, discharge planning, and survivorship care planning.
