Position Overview: We are seeking a dedicated and experienced Home Health Case Management Registered Nurse (RN) to join a comprehensive Home Health team at a local home health company in Washington. The Home Health Case Management RN will play a vital role in coordinating and managing the care of homebound patients. The Care Coordination Nurse will assess patient needs, develop care plans, coordinate services, and collaborate with interdisciplinary teams to ensure the best possible outcomes for patients in their homes.
Care Management RN Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs.
- Develop individualized care plans and coordinate services to meet patients' needs.
- Monitor and evaluate patient progress and adjust care plans as necessary.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure effective care coordination.
- Provide skilled nursing care, including wound care, medication administration, and patient education.
- Educate patients and families on managing their conditions and promoting independence.
- Document all assessments, interventions, and outcomes accurately and timely.
Home Health Case Manager Qualifications:
- Current Registered Nurse (RN) license in the state of Washington.
- Strong assessment, care coordination, and communication skills.
- Knowledge of home health regulations, Medicare guidelines, and documentation requirements.
- Excellent organizational skills and ability to manage a caseload effectively.
- Valid driver's license and reliable transportation.
