Hospital Director Case Management

This healthcare leader will report directly to the Chief Medical Officer and will be responsible for oversight of all case management activities for the 215 bed hospital, including inpatient, outpatient, and rehabilitation case management services.

The Acute Care Director Case Management will be offered room for career growth, having a dotted line to the Vice President of Case Management for the entire healthcare system.

Specific duties will include:

• Directing a highly trained team of Case Management Leaders, RNs, Social Workers, and ancillary staff, providing leadership and guidance over the organization's Case Management program.
• Collaborating with the interdisciplinary team to oversee and facilitate care coordination, discharge planning, medical necessity and clinical reviews, and assessment of proper utilization of resources.
• Promoting a positive work environment, fostering an attitude of commitment to excellence in patient safety, and supporting the team to provide excellent case management services throughout the length of stay.

REQUIREMENTS: Recent, progressive Case Management Leadership experience as a Director or Manager, RN Registered Nursing License or LCSW Licensed Clinical Social Worker in the State of Colorado.
