An award winning medical center is searching for a an experienced Inpatient Case Manager to join their Care Management team in the picturesque state of New Mexico!
The Inpatient Case Manager will have the amazing opportunity to join a state-of the art 450 bed acute care hospital. This award winning hospital has served as a valuable resource in its community and is located in the beautiful area of Central New Mexico. This medical center is one of the largest hospitals in New Mexico and is known for it's excellence in safety and cardiovascular care.
The Inpatient Case Manager will report to the Director of Case Management and will collaborate with Department Nurse Managers and Physicians. The Hospital Case Manager will play a vital role in ensuring that patients have a smooth transition from an inpatient acute care setting to a post acute rehabilitation program or skilled nursing facility. The Inpatient Case Manager will be responsible for organizing complex care, assessing patient's health insurance plans and providing necessary resources for patients being discharged. The Case Manager will act as an advocate for their patients and go above and beyond to help accurately assess their individual needs.
The Inpatient Case Manager will have the amazing opportunity to live in a location that enjoys an average of 300 days of sunshine a year!
