The Social Work Case Manager plays a vital role in the interdisciplinary team, working closely to facilitate discharge planning and address the complex care needs of patients. The Clinical Social Worker position requires expertise in navigating limited resources, complex family dynamics, mental health issues, substance use disorders, systems of care, protective services, domestic abuse, ethical concerns, and other psychosocial complexities. The Social Work Case Manager conducts mental health assessments, evidence-based interventions, cognitive function screenings, crisis interventions, safety planning, and other appropriate interventions to meet the psychosocial needs of patients. Collaboration with Nurse Care Managers, physicians, nursing staff, and other relevant personnel is essential to determine when social work involvement is necessary to address patient care needs.
Responsibilities: The Social Work Case Manager will:
- Collaborate closely with the interdisciplinary team to facilitate discharge planning and address the complex care needs of patients.
- Assess patients' mental health status and provide appropriate interventions based on evidence-based practices.
- Conduct screenings for cognitive function and implement necessary interventions.
- Perform crisis interventions and safety planning to ensure the well-being of patients.
- Administer evidence-based interventions (e.g., Screening, Brief Intervention, and Referral to Treatment - SBIRT) to patients at risk of or diagnosed with substance use disorders.
- Identify and navigate available resources to address limited resources and systems of care.
- Assess and intervene in cases involving complex family dynamics, suspected or diagnosed mental health issues, involvement with protective services, suspected domestic abuse, ethical concerns, and other psychosocial complexities.
- Collaborate with Nurse Care Managers, physicians, nursing staff, and other relevant staff members to determine the appropriate involvement of social work in addressing patient care needs.
Qualifications: The ideal candidate for this role should possess:
- A degree in Social Work or a related field from an accredited institution.
- Licensure as a Social Worker, as required by the state regulations.
- Experience in a healthcare setting, preferably working as a Social Work Case Manager.
- In-depth knowledge of mental health assessment techniques and evidence-based interventions.
- Familiarity with substance use disorder assessment and interventions, including SBIRT.
- Strong crisis intervention skills and the ability to develop safety plans.
- Proficiency in assessing and addressing complex psychosocial issues.
