The Nurse Case Manager will work in an acute care setting and provide care coordination services to patients through cross collaborating with multidisciplinary teams, evaluating options and developing patient discharge options that meet the need of each individual. The Nurse Case Manager will join the Care Coordination team, working alongside a team of specialized case managers holding their CCM Commission for Case Manager Certification. Ideally the Nurse Case Manager will have previous Case Management experience working with an inpatient population within an acute care setting. Additionally, this individual will be up to date on best practices, state, federal and hospital regulatory compliance and guidelines, have prior discharge planning assessment and utilization management experience, and hold an active registered nurse license.
