A well-respected Community Hospital in Coastal Northern California has just opened up a great opportunity for a Nurse Case Manager.
This patient-centered, Community Hospital offers a wide range of services including Emergency Care, Surgical Services, Hematology and Oncology, Laboratory, Imaging, and more.
Reporting to the Administrative Director of Patient Care, the Nurse Case Manager will work to ensure the seamless movement of patient healthcare across the continuum of care. The Nurse Case Manager will be well supported by staff nurses, nursing supervisors, nurse managers, and other healthcare professionals.
The Nurse Case Manager will be responsible for care planning, discharge planning, follow planning, and utilization review. In addition, the Nurse Case Manager will also ensure patients are receiving appropriate healthcare services both inpatient and once discharged. The Nurse Case Manager will also coordinate patient care and make recommendations to improve patient outcomes. This role will require excellent communication skills as well as the ability to collaborate with Providers and the interdisciplinary team.
