What are the responsibilities and job description for the Clinical Liaison position at Clinical Professional Connections?
Full-time at 40 hours per week
Hourly pay at $XX/hour - based off experience
Hours: 9a-5p EST M-F
Location: Clearwater, FL or Tampa, FL
Certifications: Medical Assistant (MA) or Home Health Aid (HHA) preferred but not required.
Experience in healthcare: 3 years preferred.
Education: High school diploma required; undergraduate degree preferred
Glucose Guards is a chronic disease management and remote monitoring company. The Clinical Liaison position is responsible for communicating with patients on behalf of the Medical Practices and employers with whom we partner.
Communications involve calling patients when their vital signs, including blood pressure, blood sugar, heart rate, weight, and blood oxygen levels are outside of expected ranges. These calls involve education regarding the management of chronic disease states, including diabetes, using scripts approved by clinical partners. Further, when patients are not testing or need help troubleshooting their devices, our Clinical Liaison assists with that process including sending replacement devices or testing supplies as needed.
When certain vital signs are critical, a direct escalation to the practice is necessary. All interactions need to be documented appropriately in the Glucose Guards proprietary system.
Additional administrative functions are required in this role including answering incoming calls, assisting in onboarding new patients, creating user profiles, and other tasks that may arise from time to time.
