CSO is looking for a Nurse Supervisor for our Respite Program in Gardner.

This Nurse Supervisor will provide daily administrative and programmatic coordination of HRSA Grant Clients including coordinating assignment of referrals, assisting with reporting to funders and collaborators, participation in program and policy development, providing medication monitoring and treatment for clients in CCS/Respite, work closely with staff, provide training and consultation to other nursing staff.

Our Respite services offer clients a quiet, home-like atmosphere. We provide daily assessment and brief counseling, case management, nursing support and peer support. Respite services have recovery and wellness focused groups, psychiatric consultation when needed and also provide discharge planning.

Responsibilities

HRSA RELATED RESPONSIBILITIES:

  • Provide regular coordination and administration of the HRSA Outreach project at Crisis Services.

  • Support and coordinate the assignment of referrals, and help with data for reports to funders and collaborators.

  • In collaboration with HRSA project coordinator at BFMC, support in development of program policies and procedures.

  • Collaborate with HRSA Clinical Team through attendance of regular team meetings and phone and/or text exchange to expedite HRSA team referrals.

  • Primary data entry for all HRSA referred clients including opening referrals, regular contact notes, and closing at end of service.

  • Other data entry as needed for the HRSA grant.

  • Direct support of HRSA clients as needed for program support and/or nursing expertise.

CCS/RESPITE RELATED RESPONSIBILITIES:

  • Contribute nursing expertise as part of the Mobile Crisis team on individual clients. This may include nursing assessments, referrals and consultations with primary and/or medical specialists, and consult on medical needs regarding interventions for Crisis clinicians.

  • Conduct medication reviews and monitoring as needed.

  • Participate in the development of transition of care plans for Mobile Outreach clients.

  • Review medications upon admission, confirm prescriptions and dosages, set up medication charting, administer, document and monitor medication of clients staying in CCS/Respite per ESP Policy.

  • Complete health assessments of clients in CCS/Respite and report findings to NP or MD as appropriate.

  • Monitor clients' physical status as needed, including blood pressure, pulse, temperature and blood sugar for signs of medication side effects. Report abnormal findings to prescribing clinician or MD/NP On-Call.

  • Provide training to nurses and relief nurses as necessary.

  • Assist in the development, implementation and evaluation of new or revised policies and procedures aimed at addressing existing or emerging needs.

  • Comply with DPH regulations for storage of medications.

  • Provide health education for clients as part of treatment team when appropriate.

  • Contact clients' PCP's regularly and make referral to PCP's when necessary.

  • Share observations, interactions and knowledge to develop 24-hour care plans.

  • Communicate regularly with family members, significant others, and involved providers in an effort to coordinate care.

Qualifications

Must be licensed in Massachusetts as a Registered Nurse, and have a minimum of three years' experience treating adults. Primary care experience strongly preferred. Experience treating individuals with serious mental illness and substance use disorders preferred. Must be familiar with community resources and possess good communication and interpersonal skills. Ability to establish trusting relationships through compassion, empathy and insight. Capacity to be an effective member of a team. Demonstrated sensitivity to the needs and perspectives of individuals from diverse cultural or linguistic backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software. MA driver's license and reliable transportation required.
