COMHAR is seeking a full time RN Care Manager in our Certified Behaviorial Health Clinic (day shift)

The Certified Community Behavioral Health Clinic (CCBHC) Nurse Care Manager provides care management and coordination, nurse navigation support, and health literacy education to CCBHC participants who have serious mental illness, substance use disorders, co-occurring substance use disorders, complex medical needs, and/or serious emotional disturbances.
This position is responsible for participating in and providing leadership for integrated care teams, including attending team meetings to ensure clear and effective communication among all disciplines involved in participant’s care.
Provide basic medical screenings to participants including but not limited to blood draws from patients while following strict precautionary safety procedures in order to limit the risk of contact with potentially infectious blood and other bodily fluids, as well as provide monitoring of blood pressure, body mass index, lipids, glucose and tobacco use.
Partner with participant, their care team providers, and outside supports to ensure effective care coordination and that care plan is implemented as developed and needed adjustments are made. Monitor participant's progress in treatment and assist the clinical team with evaluating the need for continued services.
Other Responsibilities:
- Complete all required documentation in a timely manner meeting Agency and division established deadlines. Attend assigned trainings; maintain necessary certifications and licenses (CPR/First Aid, Safety Care, MAT).
- Attend and participate in supervision, team meetings, and clinical rounds with medical team.
- Provide support in various program locations.
- Provide on-call coverage as needed.
Requirements
Qualifications:
- BS in Nursing.
- Current MA registration in nursing with min. 3-5 years of direct work with adults in a community setting with some work experience in the following areas:. Substance use, HIV and AIDS awareness
- Familiarity with Clozaril and REMs reports
- Providing long-acting injectable medications
- Work with the Serious Mental Illness (SMI) population
- One year of supervisory experience preferred
- Demonstrated knowledge base and judgment necessary for independent clinical decision making.
- Excellent verbal and written communications skills.
- Bi/lingual (Spanish) preferred.
- Strong interpersonal and customer relations skills required.
- Excellent organizational, time management and problem-solving skills.
- Strong computer knowledge with experience in Electronic Health Record (EHR) keeping
- Knowledge regarding psychiatric rehab and understanding of recovery model.
- Demonstrated understanding of and competence in serving culturally diverse populations.
- Commitment to advocate values and mission.
- Must hold a valid drivers' license.

Salary : $70,000 - $75,000
