Coastal Plains Community Center is seeking a Crisis Case Manager to join our Rockport/Aransas Pass team. If you are looking for an opportunity to work in an ambulatory setting while working alongside collaborative team members, then look no further.

General Description:

This position will provide crisis care services to Aransas County as well as the Aransas Pass area. Other duties to be provided will be screenings, post hospital follow-ups, crisis follow-up, assessments. This position will assist with collecting data and completing reports for the Crisis Intervention Team. This position will also maintain a small intensive caseload. Crisis worker will provide therapeutic interventions and frequent in-person contact with individuals and their families. Case management services consists of gaining access to medical, social, educational and other unmet needs. Rehabilitative services focus on positive outcomes with reduction in symptoms, increase in level of functioning through skills training. Work is community and site based. Works under limited supervision with considerable latitude for the use of independent judgment. Schedule is Monday thru Friday. Hours are primarily 8a-5p but will be flexible depending on various factors. Job requires high level of clinical skill necessary for independent judgement. This position will report to the Crisis Intervention Team Director and will also have oversight by the Rockport/ Aransas Pass Clinic Director. This position will require occasional client transport in company vehicle.

MINIMUM QUALIFICATIONS:

Bachelor’s degree from an accredited college or university with a major in a Social, Behavioral Health or Human Service area.

PREFERRED REQUIREMENTS:

1. Minimum proficiency with computer software as demonstrated by knowledge of Microsoft Applications.

2. Lives within service area.

3. Master’s degree.

4. Excellent verbal and written communication skills as demonstrated by oral and written interview.

5. Ability to communicate in Spanish.

6. Experience working with this target population (adults and children) who are experiencing crisis situations.
