About The Role
We are looking for a part-time social worker in the San Antonio, Texas area who will provide coordinated care in the home to patients of all age groups. The social worker will perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family.
Job Duties:
- Assess hospice patient to identify psychosocial, financial, environmental, and community needs as evidenced by Plan of Care documentation, clinical records, team meeting minutes and community resource referrals
- Attend interdisciplinary group meetings, participate in the patient care planning process, and collaborate with Interdisciplinary Group to promote coordination of patient care
- Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes
- Meet mandatory continuing education requirements of the agency/licensing board
- Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements
- Promote Agency philosophy and administrative policies to ensure quality of care
- Accept clinical assignments that are consistent with the education and competence to meet the needs of the patients
Desired Profile:
- Must be licensed by the state of Texas as a Licensed Clinical Social Worker (LCSW)
- Masters Degree in Social Work from a state accredited program
- Must have 1 year of direct patient care experience in acute or sub-acute care
- Must be comfortable dealing with and discussing dying, grief and end-of-life issues
- Must be able to travel to jobs per each assignment with valid driver’s license
- Ability to work with technology and EMR to follow/update patient plan of care
- Persistent, positive attitude, excellent interpersonal skills
