Position Summary: Lifestyle Coordinator plans, organizes,
facilitates and directs recreational activity programs for residents in the Assisted
Living Community. Prepare for events by obtaining and organizing necessary
materials for activities. Directs and
organizes such activities as sports, dramatics, games and arts and crafts to
assist residents to develop interpersonal relationships, to socialize
effectively, and to develop confidence needed to participate in group
activities. Regulates content of program
in accordance with individualized patient capabilities, needs and
interests. Organizes and coordinates
special outings and may accompany residents on outings. Plan and document a monthly activity
calendar. Participate in assessing and
documenting progress charts and periodic reports for medical staff and other
members of the treatment team reflecting resident reaction and evidence of
progress or regression. Network with
community organizations, businesses, schools and individuals to form a solid
volunteer program. Participate in
planning and implementing internal and external Community events.

- Leads recreational activities including the 7 dimensions of wellness, intellectual
and educational activities, arts/crafts, Community outings, etc.
- Coordinates or implements a volunteer
program.

- Demonstrates excellent organizational,
planning, and time management skills.
- Leads large groups
- Conduct one on one activities with
Residents
- Plan and execute resident outings
- Maintains resident confidentiality and

Work Environment: This
job operates in a professional office or home office environment depending on
location. This role routinely uses standard office equipment such as computers,
phones, photocopiers, filing cabinets and fax machines. Operate a car or
company vehicle for required travel.
Physical Demands: The physical demands
described here are representative of those that must be met by an employee to
successfully perform the essential functions of this job.
The physical demands of this
position include;
- Standing, sitting, walking, changing of
position, reaching, extending hands & arms, reach across midline,
crouching, balancing, kneeling, stooping, bending, lifting 25 lbs. from sit to
stand & push, pull, lower 100 lbs. and push resident in wheelchair 350 lbs.
Do not
attempt to lift residents or support entire weight of resident.
Lower
resident to floor in the event of fall.
Utilize two man lift procedure
Utilize
required safety equipment
Sensory Demands:
•
Auditory: to hear and understand verbal request,
•
Visually: be able to read necessary information to complete
work tasks,
•
Verbally:
communicate with customers and others on your team
Safety Equipment:
•
When transferring a resident, gait belt, lumbar
support, two man lift policy and use of mechanical lifts in noted
facilities.
•
Use of PPE is required when appropriate to
prevent exposure to bloodborne pathogens.
•
Use of seat belt when driving or riding in a
vehicle, no use of cell phone while driving.
•
Mandatory reporting of all work-related injuries
within 12 hours to supervisor with participation in required documentation.
•
Participation in our drug testing procedures is
a mandatory part of employment with this Community.
OSHA Exposure: Category 1,
possibility of exposure on daily basis. Required
use of personal protective equipment
Position
Type/Expected Hours of Work: This could be a full-time or part time position.
Hours can vary depending on coverage and demands of the community.
Travel: Some travel will be required to provide offsite activities
for residents and attend organizational meetings. This position will also
be expected to assist with transportation of residents to and from appointments.
Qualifications/ Education/Experience/Skills:
Education :
- Minimal
high school graduate and minimum age of 18 years of age
License/Certifications:
- Current Driver’s license required
