Overview

This position works with at risk clients, their family, the medical care team, and Family Services program staff in identifying medical, behavioral, and social needs and providing case management interventions to achieve optimal patient outcomes.  

 

Responsibilities

1. Works as part of an interdisciplinary care team to manage the needs of at risk clients to improve access to care and improve outcomes.

a. Work closely with clients experiencing medical, behavioral and social conditions impacting their health achieve positive outcomesb. Assist clients in following through with the care plan in cooperation with the interdisciplinary care team.c. Provide patient centered education on topics pertaining to wellness and prevention.

 

2. Case Management

a. Coordinate medical, behavioral, and social health care needs and link the patient to needed resources (transportation, financial assistance, etc.) in order to achieve good care outcomes and/or successfully manage condition. b. Assist clients by answering questions and teaching how to access needed community services/resources in order to enable clients to be self-sufficient.c. Engage in home and/or office visits of eligible clientsd. Provide motivation and supporte. Develop and maintain relationships with community agencies relevant to program functions or referral needs.

 

3. Effectively manage caseload by providing timely interventions, providing quality services, achieving positive outcomes, and maintaining productivity levels.

 

4. Maintain documentation of services provided, interventions, and referrals, as well as follow up care and progress toward outcomes in accordance with program requirements and guidelines.

Qualifications

General Development:

1. Requires more advanced organizational skills, in order to organize projects or the work of others. 2. Job duties require the ability to work independently and as part of a team. 3. Job responsibilities require individual development of priorities for effective performance of duties, including re-prioritization in response to changes in circumstances. 4. Employees are expected to devise effective solutions to situations encountered based on general goals and objectives of the function. 5. Work requires the incorporation of departmental/functional process into the overall functioning of the organization. 6. Duties require drawing conclusions using inference and logic, which may be different than the conclusion that could be drawn by others.

 

Professional and Technical Knowledge:

1. Performance of the duties and responsibilities of the job require the equivalent of formal training in behavioral sciences and case management interventions, usually in the form of a major in social work or related field as part of a Bachelor’s degree program. 2. Possess a valid Washington State driver’s license with the ability to travel to perform home visits and other work related activities.

 

Technical Skills:

1. Basic knowledge on using computers and the ability to work in a Windows environment using Microsoft Work, Excel, PowerPoint, and Outlook. 2. Ability to enter and manage data into a centralized electronic health records.

 

Communication Skills:

1. Job duties require the employee to effectively communicate their opinions and extrapolations of information they collect and synthesize/analyze. 2. Employees are expected to exercise tact and diplomacy in the resolution of mild conflict or disagreements. 3. Job duties require the compilation and analysis of information prepared in effective written form, including correspondence, reports, articles, or other documentation. 4. Duties involve others to take actions or to behave in specific way desired by the employee. 5. This position requires bilingual skills in Spanish and English.

 

Blood/Fluid Exposure Risk: According to this position this is a Category I

1. Category I: Tasks routinely involve a potential for mucous membrane or skin contact exposure to blood, fluids or tissue. Use of personal protective equipment (PPE), when appropriate, is required.2. Category II: Usual Tasks do not involve exposure to blood, body fluid, or tissues but may require performing Unplanned Category I tasks.3. Category III: Tasks involve no greater exposure to blood, body fluids or tissues than would be encountered by a visit. Category I tasks are not a condition of employment.

 

Physical Demands & Vision Requirements:

1. Ability to stand, walk, sit, use hands, fingers, and talk frequently. 2. Ability to lift up to 25 pounds 3. Close vision (clear vision at 20 inches or less)4. Distance vision (clear vision at 20 feet or more)5. Ability to adjust focus (ability to adjust the eye to bring an object into sharp focus)

 

Travel:

1. Frequent travel to various clinic sites is required.2. Must possess a valid driver’s license and meet company insurance liability standards to drive company vehicles.

In house credit includes for one eye exam every plan year. $200 hardware credit for employee and dependents for prescription hardware (glasses, contacts, prescription sunglasses). Employees and dependents also receive one additional hardware credit per year at CBHA’s cost 20% (used for non-prescription sunglasses, contact lenses, or a second pair of prescription glasses). $35 copay is waived for preventative services

403 (b) Retirement Plan: 

Safe Harbor Match: Employees are eligible to receive a Safe Harbor Match of 100% up to 4% of the employee’s total salary. Employee is full vested in Safe Harbor Match from day 1. Contributions are subject to IRS guidelines.

Non-Elective Contribution- Provided the employee has met enrollment qualifications, employee will also receive 4% employer contribution of employee’s total salary at the end of the Plan Year at the discretion of Board of Directors.

 

Vacation (Personal Leave): 

 

Year 1

40 hours front loaded after six (6) months of employment. Accrue at the rate of .0038per hour paid not to exceed a per pay period accrual of .3076 hours for the rest of year one (1). Vacation buyout paid hours are excluded from accrual

 

Year 2 

Accrue at the rate of .0442 per hour paid not to exceed a per pay period accrual of 3.53hours. Vacation buyout paid hours are excluded from accrual.

 

Year 3

Accrue at the rate of .5576 per hour paid not to exceed a per pay period accrual of 4.46 hours. Vacation buyout paid hours

 

All employees receive 8 vacation hours for a Wellness Day on their Anniversary Date each year.
