PrimaryOne Health is currently seeking a Homeless Program Case Worker. This is a Non-exempt position reporting to the Senior Director of Community Services with a primary responsibility to determine eligibility, refer to services, provide outreach, and process individuals within a specific population as identified by PrimaryOne Health process and procedures. This position will be located at our E. Main Street Health Center however be our in the community providing resources as needed.


Essential Job Responsibilities:

  • Provide crisis assistance to help facilitate: securing food, shelter, clothing and assisting clients in obtaining a primary care medical home.

  • Assist clients completing initial entry forms and maintains client records.

  • Coordinate appointments, transportation and other social or personal services as needed. Responsible for client follow-up after broken appointments and/or incomplete service delivery.

  • Assess client needs for services such as: crisis intervention, service planning/implementation; confers with medical, social service, shelter and community personnel to gain insight into client’s wellness; advocates on client behalf. Monitors progress as needed.

  • Participates in Quality Improvement activities.

  • Establishes eligibility for services.

  • Provides referral to rehabilitative services as needed; this includes general symptom management and support, community living skills training, employment services, medication education/monitoring and linkage to community services.

  • Complete all necessary documentation in a timely manner as required by PrimaryOne Health service policy and procedures, regulatory requirements, or any other stipulation of service agreement.

  • Moderate travel required.

  • Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.

  • Other duties or special projects as assigned.

Required:

  • High school diploma or equivalent; some college preferred in behavioral, human services, or related field.
  • One (1) year of community service experience preferred.
  • Must have a valid driver’s license and automobile insurance that meets company threshold.


Employment with PrimaryOne Health is contingent on new hires completing full COVID-19 vaccination within 45 days of start date to maintain employment (unless you request and receive an exemption).
