We are looking for a compassionate, reliable, and customer-service oriented Licensed Vocational Nurse (LVN) to join our home health team.
Position: Patient Care Coordinator (PCC) (LVN)
Reports to: Clinical Manager
Job Description Summary
Responsible for coordinating home health services for patients which includes establishing and maintaining the patient’s plan of care, establishing and maintaining positive relationships with patients/family members and related referral sources.
Essential Job Functions/Responsibilities
1. Provides professional nursing care by utilizing all elements of the nursing process.
2. Coordinates home health services for patients with the interdisciplinary group to ensure the home health plan of care is met.
3. Maintains positive working relationships with colleagues, patients, patient's families, and community partners.
4. Promptly responds to patient care needs, requests and concerns.
5. Assists with triaging clinical calls that are received from patients, families, and community partners.
6. Assists with requesting new/adjusted physician orders.
7.. Becomes compliant with all departmental standards, policies and procedures, training and education, workforce management, disaster planning, effectively managing and prioritizing work tasks, attendance, adherence, and quality.

Qualifications
1. Two or more years related experience in nursing and one year home healthcare experience required.
2. Complies with acceptable professional standards and practice.
3. Ability to tactfully deal with patients, patient's families, and the community.
4. Time management and organizational skills working in a fast paced office environment.
5. Demonstrates good communications and public relations skills.
6. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
